QUICKmed Care Management is a new program created for our Medicare patients to help make health care easier!

Our job is to help you stay healthy and out of the hospital. We can do this by making sure you have the information you need, the medicine you need, and the follow-up care you need.

 

Care Management Benefits:

  • Give you 24/7 phone access to a care team member to help with urgent issues that come up when we are closed.
  • Help you schedule needed appointments with your primary care provider, specialists or for outside testing.
  • Help you get your medication and refills.
  • Help you get needed medical supplies.
  • Create a patient centered care plan that includes all of your medical problems, medications, care team and health goals.

 

Our team of care managers will check in on you between office visits and can be can EXTRA resource for you!
