DOT Physicals
Commercial drivers must be examined by a medical examiner listed on the FMCSA National Registry of Certified Medical Examiners. The examiner completes the Medical Examination Report (Form MCSA-5875) and, if the driver qualifies, issues the Medical Examiner's Certificate (Form MCSA-5876) — the card drivers carry.
A certificate can be issued for up to 24 months. Drivers with conditions that need closer monitoring, such as high blood pressure or diabetes, often receive shorter certification periods.
Tell your drivers to bring photo ID, a full medication list with dosages, and their glasses, contacts, or hearing aids. Drivers managing a condition should bring recent documentation from their treating provider — it prevents repeat visits.
T-8 Physicals for Ohio School Transportation Drivers
Ohio school bus and van drivers need an annual T-8 physical — formally the School Transportation Driver Medical Examination — required under Ohio Administrative Code 3301-83-07. This is not the same as a federal DOT exam, and the two are not interchangeable.
What transportation supervisors should know:
- Physicals for the coming school year can be completed starting May 1.
- Each driver must pass the T-8 before September 1 of the new school year.
- The T-8 has two parts — the Driver/Employer form and the Physician's form. They must be attached to each other or printed back-to-back to be valid.
- The employer supplies the paper form and enters the results into the state system.
Scheduling drivers in May and June, rather than late August, keeps your roster qualified before the first day of school.
